Welcome to Quick Test Health Services of Tucson, a locally owned, CLIA-certified medical testing laboratory located at 1310 W St. Mary's Rd, 2nd Floor, Tucson, Arizona 85745. These Terms and Conditions ("Terms") govern your use of our laboratory testing services, including but not limited to drug testing, STD testing, DNA testing, blood work, TB testing, and N95 fit testing in Tucson and surrounding Pima County communities. By accessing our services, scheduling an appointment, or using our website at www.quicktesttucson.com, you acknowledge that you have read, understood, and agree to be bound by these Terms and Conditions.
Quick Test Health Services of Tucson is a locally owned business drawing on extensive healthcare experience to provide professional, confidential, and accurate medical testing services to individuals, businesses, legal entities, and healthcare providers throughout Tucson and Pima County, Arizona.
1. Definitions
"Client" or "You" refers to any individual, employer, legal entity, or organization using Quick Test Health Services of Tucson's testing services.
"Services" refers to all laboratory testing, specimen collection, and related services provided by Quick Test Health Services of Tucson.
"Third Party" refers to employers, attorneys, courts, or testing companies that order testing on behalf of a donor or employee.
2. Services Offered
2.1 Medical Testing Services in Tucson, AZ
Quick Test Health Services of Tucson provides testing services as a CLIA-certified collection facility, including:
Drug Testing Services Tucson: DOT and non-DOT testing (urine, hair, breath alcohol) for employment, court-ordered testing, and accident screening.
STD Testing Tucson: Confidential sexually transmitted disease testing (HIV, Chlamydia, Gonorrhea, etc.) without insurance or referral required.
DNA Testing Tucson: Paternity, maternity, sibling, grandparentage, and legal DNA testing.
Blood Work Tucson: Hormone panels, thyroid testing, CBC, metabolic panels, and health panels.
Other Services: TB Testing, N95 Fit Testing, and Mobile Testing Services.
2.2 Testing Standards and Certification
Quick Test Health Services of Tucson is a CLIA-certified collection facility operating in full compliance with federal regulations and Centers for Medicare & Medicaid Services (CMS) standards. We partner with SAMHSA-certified laboratories for all drug testing analysis, and all results are verified by licensed Medical Review Officers (MRO) as required by Arizona drug testing laws and DOT regulations.
Arizona Drug Testing Compliance: We comply with Arizona Revised Statutes (ARS) Section 23-493.03(5), which requires drug testing to be conducted only at laboratories certified by the United States Department of Health and Human Services or the College of American Pathologists.
3. Appointments and Walk-In Services
3.1 Scheduling and Availability
Quick Test Health Services of Tucson welcomes both walk-in clients and scheduled appointments. Most appointments take approximately 15 minutes, and many rapid screening tests provide results before you leave.
For Services Purchased Directly Through Quick Test of Tucson:

For Third-Party Collection Services:
If your test was ordered through another company (employer, attorney, testing service provider, etc.):

Appointment and cancellation policies are set by the ordering party

3.2 Cancellation and No-Show Policy
For Services Purchased Directly Through Quick Test of Tucson:
Quick Test Health Services of Tucson operates on a non-refundable payment policy. All payments made for services are final, non-refundable, and automatically convert to a non-expiring service credit if the original service is not rendered.
Prepayment is required for all scheduled appointments. This does not apply to third-party testing services.

Cancellations with 24+ hours' notice: Payment is held as a non-expiring service credit. You may reschedule the same service at no extra charge or apply the credit to any other service.

Same-day cancellations: Payment is held as a non-expiring service credit. You may reschedule once at no extra charge. Subsequent same-day cancellations require a new prepayment.

No-shows: Payment is forfeited unless you call within 24 hours of the missed appointment, in which case it converts to a non-expiring service credit. A new prepayment is required to reschedule.

Walk-in services: Payment due at time of service. No prepayment required.
For Third-Party Collection Services:
Cancellation and rescheduling policies are determined by the ordering organization. Contact the party who scheduled your test to cancel or reschedule.
Court-Ordered and DOT Testing: Missed appointments may be reported to the ordering authority. Failure to appear may have legal or employment consequences.
4. Payment Terms and Pricing

4.2 Payment Policy
Prepayment is required for scheduled appointments. Walk-in clients pay at the time of service. We do not bill insurance directly, but we can provide itemized receipts for you to submit to your insurance provider for potential reimbursement.
A 3% processing fee will be added to all credit card transactions.
4.3 Pricing and Transparency
All service pricing is clearly posted at www.quicktesttucson.com and at our Tucson facility. We are committed to transparent, upfront pricing with no hidden fees. Final pricing will be confirmed before services are rendered.

5. Specimen Collection and Testing Procedures
5.1 Collection Standards
All specimen collection is performed by trained, certified collection specialists using federally-approved protocols, in compliance with:

5.2 Direct Observation Collections
In accordance with 49 CFR Part 40.67, direct observation collections may be required when there is reason to believe a donor may attempt to tamper with a specimen. Direct observation is conducted by a same-gender collection specialist.
5.3 Specimen Validity Testing
We reserve the right to reject specimens showing signs of adulteration, substitution, or tampering, including abnormal temperature, appearance, pH, specific gravity, or creatinine levels.
Refusal to Test: Refusing to provide a specimen, attempting to adulterate a specimen, or leaving before completing the collection process constitutes a refusal to test and will be reported to the ordering party. A refusal to test is treated the same as a positive result for employment purposes.
5.4 Privacy During Collection
Donors cannot leave the collection facility between providing the specimen and completing chain of custody documentation, in accordance with DOT protocol.
6. Test Results and Reporting
6.1 Turnaround Times

Rapid/Instant Tests: Approximately 15 minutes (screening only)

Standard Lab Work / Negative Drug Tests: Typically 24–72 hours

Specialized / Confirmed Positive Tests: 3–5 business days or longer
All turnaround times are estimates and are not guaranteed. Quick Test Health Services of Tucson is not responsible for delays caused by laboratory backlogs, shipping issues, holidays, or other circumstances beyond our control.
6.2 Result Notification
Results are released to the party specified at the time of collection: the donor, employer, attorney, court, or healthcare provider. Results are communicated via secure email, phone, or our online portal.
6.3 Confidentiality of Results
In compliance with HIPAA and Arizona Revised Statutes Section 23-493.06, test results are strictly confidential and released only to authorized parties. Unauthorized disclosure may result in legal penalties.
6.4 Accuracy and Medical Review
All positive drug test results undergo confirmation testing (GC/MS or LC/MS/MS) as required by Arizona law. Confirmed positive results are reviewed by a licensed Medical Review Officer (MRO) before being reported.
7. Arizona Drug Testing Law Compliance

Arizona law permits reasonable suspicion and random drug testing when conducted according to a written policy. We provide immediate testing for reasonable suspicion situations and can administer random testing programs for Tucson-area businesses.

Quick Test Health Services of Tucson conducts DOT drug and alcohol testing for safety-sensitive employees under: FMCSA, FAA, FTA, FRA, PHMSA, and USCG.
8.2 DOT 49 CFR Part 40 Compliance
All DOT testing complies with 49 CFR Part 40, including use of federal CCF forms, DOT 5-panel testing, SAMHSA-certified lab analysis, MRO review, split specimen procedures, and proper chain of custody documentation.
8.3 FMCSA Clearinghouse Reporting
All DOT drug and alcohol violations are reported to the FMCSA Drug and Alcohol Clearinghouse as required by federal law.
9. Legal and Court-Ordered Testing
9.1 Court-Admissible Testing
We provide legally defensible, court-admissible testing for custody disputes, probation and parole monitoring, CPS cases, family and criminal court proceedings, immigration testing, and legal paternity establishment.
9.2 Chain of Custody Requirements
All legal testing includes photo ID verification, witnessed specimen collection, sealed tamper-evident containers, complete documentation, and certifications for court presentation.
9.3 Expert Testimony
Collection specialists and partnered laboratory directors are available for expert witness testimony in Arizona courts. Additional fees apply.
10. Privacy, Confidentiality, and Data Security
10.1 HIPAA Compliance
Quick Test Health Services of Tucson is a HIPAA-covered entity and maintains full compliance with HIPAA 1996. We implement administrative, physical, and technical safeguards to protect your protected health information (PHI).
10.2 Information We Collect

11. Direct-to-Consumer (DTC) Lab Testing and Blood Work
11.1 No Medical Advice
Quick Test Health Services of Tucson is NOT a physician's office. We do not provide medical advice, diagnosis, treatment, or interpretation of lab results. You must consult a licensed physician for interpretation and treatment.
11.2 Physician Order and Supervision
We provide the order for you through our authorized ordering provider — no doctor referral needed.

Lab tests are not a substitute for a comprehensive examination by a healthcare provider.

It is your sole responsibility to consult your own licensed doctor for interpretation and action on your results.

The cost of the authorized ordering service is included in the total price of your test.
11.3 Limitation of Liability
Quick Test Health Services of Tucson, including its officers, agents, employees, Medical Director, and Nurse Practitioner, shall not be liable for: inaccurate, delayed, or misinterpreted test results; errors or delays caused by third-party laboratories, MROs, or couriers; or any damages resulting from use or misuse of test results.
Quick Test Health Services of Tucson acts solely as a specimen collection site and facilitator.
11.4 Third-Party Laboratory Relationships
We partner with SAMHSA-certified and CLIA-certified third-party laboratories for specimen analysis. We are not liable for laboratory processing delays or analytical errors beyond our direct control.
12. Client Conduct and Facility Rules
12.1 Professional Environment
All clients, donors, and visitors are expected to treat staff and others with respect, follow staff instructions, and comply with collection protocols.
12.2 Refusal of Service
We reserve the right to refuse service to anyone who exhibits threatening or abusive behavior, attempts to tamper with specimens, refuses to provide proper identification, or has outstanding unpaid balances for non-court-ordered testing.
12.3 Zero Tolerance for Tampering
Any attempt to tamper with specimens will result in an immediate refusal to test report, permanent ban from our facility, notification to relevant authorities, and potential criminal prosecution under Arizona law.
13. Website Terms and Online Services
13.1 Website Use

16. Class Action and Arbitration Waiver
All claims must be brought individually. Disputes that cannot be resolved informally must be resolved by binding arbitration in Pima County, Arizona, under the rules of the American Arbitration Association.
17. Severability and Entire Agreement
If any provision of these Terms is found invalid or unenforceable, the remaining provisions continue in full force. These Terms, along with any service-specific consent forms, constitute the entire agreement between you and Quick Test Health Services of Tucson.
18. Governing Law and Contact Information
These Terms are governed by the laws of the State of Arizona. Any legal action shall be brought in the courts of Pima County, Arizona.
